When moving from Humana to Blue Cross (Large Group), since Humana lags claim information by 90 days, if for example we are shooting for a 9/1 start date, we will need claims from your last anniversary through the end of May. June and July would not be available and would not be required.
In order to get those rates firmed up, we will need you to email Rebecca Bushke <rbushke@humana.com> and request claims data again as we did before and have her release info for Months including April & May. Note: The rates you have now are standard and could adjust.
Please note the following Blue Cross Requirements:
- Underwriting reserves the right to adjust the rates if enrollment changes by more than +/- 10% from quoted enrollment
Source documentation of the following is required at enrollment for this proposal to be valid:
- Current SUTA detailed with FTE, PTE, ineligibles, job titles…etc.
- Most Recent Billing (within 60 days of the effective date)
- Group Health Questionnaire completed, signed and dated by the group leader within 30 days of the effective date
- Current claims experience including large claimant information within 90 days of the effective date as allowed by law
