• The employee and domestic partner represent that they meet each of the following identified criteria:
  • They are each other’s sole domestic partner and intend to remain so indefinitely.
  • They are of the (same or opposite) sex and neither one is married to someone else.
  • They are at least 18 years of age and mentally competent to consent to a contract.
  • They are not related by blood to a degree of closeness that would prevent legal marriage
  • They are jointly responsible for each other’s common welfare and financial obligations
